Twisted is where your mind is if you run this marathon, and also what the elevation map looks like!  Certainly a challenge, and the medal is ceramic which hangs on a regular rope. Robert Rayder, maniac #1340, who took this pic says it has an intended “folk art” feel to the medal.  Their WEBSITE.
